An Initial Appointment is appropriate for any animal who is a first-time patient, has a new concern, or has not been seen in over one year.

Equine, Canine, or Feline Adjustment is appropriate for established patients on an active or maintenance plan and seen in the past year.

Payment options: cash, check, Zelle, Venmo, or credit card. Payment is due in full at the time of service.

*A veterinarian referral must be on file for each animal before starting care. 

Notes will be provided to you and your referring veterinarian, via email, after each visit. This allows your animal’s primary care provider to stay informed of their progress and monitor care. 

If you wish for notes to be sent to a trainer or other animal care provider just provide their email and we will keep them in the loop as well. We are big fans of collaborative care for animals. 

Smooth Transitions Chiropractic, PLLC is mobile and we come to you. Trip fees are based on the distance traveled and the number of animals being seen.

Depending on the distance, the trip fee may be waived if three or more animals are receiving adjustments during one visit in a single location. Please inquire about opportunities to waive this fee. 

A minimum of 24 hours notice is required to reschedule or cancel an appointment to avoid payment in full.

We need to have your Veterinarian Referral Form on file before scheduling your first appointment. 

To expedite your first visit, please send your completed forms attached in an email and include your animal’s name in the subject line of the email “(Your animal’s name) Vet Referral”.
